Clinical Assistance Quality Assurance Recognition

A Clinical Assistance Quality Assurance Recognition is awarded to stores or clinics with one or more employees who already possess an IPFH Fitting Quality Assurance Recognition and meet the following criteria:
- The applicant organization’s authorized representative states
- that the applicant organization has chosen to commit to supporting the medical community through its services as a preventive foot health provider; and
- that to the extent that any Integrated Solution™ component products – sockwear, inserts and footwear – have been awarded an IPFH Product Quality Assurance Recognition, the applicant organization and its employees will exercise its/their best efforts to design each patient’s personal Integrated Solution™ using only component products that have been awarded an IPFH Product Quality Assurance Recognition.
- The applicant organization provides a list of the names and addresses of the 5 doctors who have referred the most number of patients to the applicant organization over the past 12 months accompanied by a statement authorizing IPFH to contact those doctors as references in support of the organization’s application
- Copies of any educational programs made by the applicant organization to the professional community over the past 12 months (agenda outlines, PowerPoint presentation slides, copies of literature dropped-off, blank organizational Rx forms, blank TSD documents provided to the professional community will be submitted to IPFH for review
- Copies of the applicant organization’s blank patient examination forms and a statement as to the applicant organization’s record keeping methods and practices will be submitted to IPFH for review
- Copies of six different redacted (eliminating patient names and addresses only) actual follow-up communications submitted to referring physicians after serving the patients they referred to the applicant organization will be submitted to IPFH for review.
